Position Description

Pride Health is seeking an RN (Home Health) to join our team in Fort Mill, SC, 29707. This is a 13-week contract with the potential for extension. Both local and travel candidates are encouraged to apply.

Position Summary:

The primary responsibility of the Registered Nurse (RN) in Home Health is to provide skilled nursing care to patients in their place of residence, following physician orders and the individualized plan of care. The RN performs comprehensive patient assessments, administers treatments, and monitors clinical progress while promoting patient independence and quality of life. The RN coordinates care with the interdisciplinary team, educates patients and families on health management, and ensures all services are delivered in compliance with Medicare, state, and federal regulations. This role requires strong clinical judgment, autonomy, and a commitment to compassionate, patient-centered care.

Job Responsibilities:

  • Conduct comprehensive assessments to identify physical, psychosocial, and environmental needs of patients.
  • Develop, implement, and evaluate individualized Plans of Care (POC) to ensure quality and continuity.
  • Educate patients and families on care routines, medications, safety, and self-management strategies.
  • Document all visits, changes in condition, and care provided in compliance with clinical and regulatory standards.
  • Supervise HHAs, STNAs, and LVNs to ensure quality and consistency of care delivery.
  • Coordinate care with interdisciplinary team members and arrange referrals when appropriate.
  • Provide timely on-call support and effective communication with patients, families, and healthcare providers.

Qualifications:

  • OASIS Experience - Required
  • Current state license as a Registered Nurse - Required
  • CHPN certification - Preferred
  • Proof of current CPR certification - Required
  • Current driver's license - Required
  • Must have reliable transportation and insurance - Required
  • 1-2 years of experience as a Registered Nurse in a clinical care setting - Required
  • 2+ years of Home health experience - Required.

Additional Information:

  • Location: Fort Hill, SC
  • Schedule: Monday – Friday, Days flexible hours based on caseload
  • Contract Length: 13 weeks
  • Travel Pay (Gross Weekly): $2,160
  • Hourly W2 (Taxable): $22.85
  • Stipend (Non-Taxable): $31.15/hour (split: $19.25 housing + $11.90 meals)
  • Local Pay Rate: $40 – $42/hour

Offered pay rate will be based on education, experience, and healthcare credentials.
