Position Description

Job Title: Registered Nurse (RN) – Home Health

Location: Rock Hill, South Carolina

Facility Type: Home Health

Contract Duration: July 28, 2025 – October 27, 2025

Shift: 3x12 Days (36 hours/week)

Weekly Pay: $1,900

Position Overview:

We are seeking a compassionate and experienced Home Health Registered Nurse (RN) for a 13-week daytime assignment in Rock Hill, SC. This role involves providing in-home skilled nursing care to patients recovering from illness, surgery, or managing chronic conditions. Ideal candidates are confident working autonomously and managing a flexible home visit schedule.

Responsibilities:

Conduct patient assessments and develop individualized care plans.
Administer medications and treatments as prescribed.
Educate patients and families on health conditions and home care best practices.
Maintain accurate and timely documentation in EMR systems.
Coordinate with interdisciplinary teams to ensure comprehensive care.
Travel to patient homes within the Rock Hill service area.

Requirements:

Active RN license in South Carolina (or compact license).
Minimum 1 year of recent Home Health RN experience.
Proficiency with OASIS documentation strongly preferred.
Current BLS certification.
Valid driver's license, reliable transportation, and proof of auto insurance.
Strong time management and organizational skills.

Start Date: July 28, 2025
End Date: October 27, 2025
Shift: 3x12 Days (12-hour day shifts, 3 days/week)
Weekly Pay: $1,900 (gross)
